What is Continuity of care?

Continuity of care means seeing the same doctor or healthcare professional consistently, so they get to know you, your health history, and your preferences. It helps to build trust and improve the quality of your care.

  Why is this important?

  • Better outcomes: Your doctor understands your health history, making diagnosis and treatment more accurate.
  • Fewer repeats: You avoid repeating your story or tests unnecessarily.
  • Personalised care: The care you receive is tailored to you, not just your condition.
  • Improved safety: Continuity reduces the risk of missed diagnoses or conflicting treatments.

 How we support Continuity

We aim to offer continuity where possible, particularly for long-term or complex health needs.

  • You have been allocated a usual GP and we will aim to book you in with this GP where possible.
  • Your named GP is part of a small Team which includes Male and female GP’s.
  • The team shares clinical notes securely to support joined-up care when your usual GP isn't available.

 How you can support Continuity

  • Ask to book with your usual GP
  • For routine appointments, try to plan ahead so we can match you with the same clinician whenever possible
  • Use our online services to make repeat booking easier

When Continuity isn't always possible

In urgent cases, or during busy times, you may need to see another GP or Healthcare professional.  Rest assured, your medical record is always available to ensure safe , effective care.
